Gelmir close to the Seethewater Terminus Site of Grace.The Fort is surrounded by Fire Monks and Flame Chariots.You can avoid most of the enemies outside the fort around the wooden barricades by quickly riding towards the fort's entrance but there is a single Fire Monk near the steps that you should take out, lest it follow. I find it easier to go from Erdtree Grazing Hill, go north and past Wyndham Ruins and you should reach Seethewater River, and then you can continue along the river to the terminus From the Site of Grace, drop down a small ledge to find 2 Leyndell Soldiers, one patrolling and one sitting in a room to the right.Continue onwards and drop down a hole to reach the main area of the cave with a poisonous floor and many Servants of Rot.Watch out for Skeletal Slimes that drip from the ceiling.
